The Dental Club In-office Dental Plan

This program was developed, because our office provides innovative quality dental care at a reasonable fee; however, the cost of health and dental care continues to increase. Our efforts are to enable good dental health and assist our patients with rising dental costs. We have developed an "in-house" dental benefits plan available to patients who cannot or do not obtain any other type of coverage. Our dental program provides a wide scope of benefits at an affordable price to help minimize the cost of your dental needs. As a courtesy we offer the discounts on ALL cosmetic services and products our office provides.

Advantages of the Program:

• No deductible.
• No exclusions for pre-existing conditions.
• No claim forms or questionnaires to fill out.
• No yearly maximum for benefits.
• No exclusions for Cosmetic Dentistry

Adults Annual Fee $380.00

Children Annual Fee (13 years old or younger) $325.00

Requirements of the Program:

• Fees are guaranteed for one year.
• Initial annual fee is nonrefundable.
• Payment is due at the time services are rendered.
• It is the patients’ responsibility to make and keep their appointments.
• You must pay renewal premium by due date or you do not qualify for percentage increase.
• Our program is for you and is not transferable to another party. However, you can add dependents for an additional fee of $325
